A great deal of research is being conducted on treatments for FH and lowering LDL levels. The appropriate treatment is determined on an individual basis through consultation between the healthcare provider and the patient.
In 2025, a review was conducted as part of the update to the European directive. Figure 2 shows the effect of various treatments on LDL. In FH, combination therapy is generally used.
A 20-year follow-up study conducted at Amsterdam UMC shows a positive effect of treatment for FH patients. Among treated children, the incidence of cardiovascular disease and mortality drops significantly to nearly 0%. For FH patients, it is important to start the appropriate treatment in a timely manner. This is due to prolonged exposure to elevated LDL levels (see also risks).

Een kalkscore (of CT-calciumscore) is een meting van de hoeveelheid calcium in de wanden van de kransslagaders van het hart, vastgesteld met een CT-scan, die aangeeft hoeveel aderverkalking (plaque) er is en daarmee een voorspeller is van je risico op hart- en vaatziekten zoals een hartinfarct. Bij FH patienten kan de kalkscore het risico onderschatten, omdat bij hen sprake kan zijn van non-calcified plak, als gevolg van een lange blootstelling aan LDL, met name bij jonge patienten (<45 jaar). Behandeling bij FH op vroege leeftijd is dus belangrijk ondanks CAC-score.
